Following Surgery/Hospitalization:

		Discuss therapy with your Case Manager.
		Provide your Case Manager with information about Wellness Care.

Your Case Manager will arrange for your therapy with Wellness Care prior to your discharge
from the hospital.

Free Screening:

Call us for a free balance and gait screen or have your assistive device checked for proper
alignment by our professionally licensed and trained staff.

Who Benefits From This Program:

In Home Rehab is designed for the general community – especially in areas with a high
proportion of senior citizens likely to benefit from rehabilitation services but who may not feel
comfortable traveling to a clinic for needed therapy services. Some of our patients simply
prefer to be seen in the comfort of their homes. This program is also intended for patients
who are truly homebound who also require skilled services such as nursing or wound care.
